Your First Visit

On your first appointment, it’s important to be prepared. Please go through the checklist below:

Patients should bring the following:Your First Visit

  • Insurance card
  • Authorizations (if applicable)
  • Photo ID
  • Patient Forms

Some patients should also bring:

  • MRI and written reports
  • Original scans and written reports of each
  • X-rays and written reports
  • Surgical reports
  • Other physician office addresses and phone numbers

Please present your completed forms to the receptionist upon arrival. We recommend that you arrive early in order to complete any additional paperwork. If you are more than 15 minutes late for your appointment you may be asked to reschedule. If your insurance company requires a referral, please make sure you have this in place before the day of your visit.
